The Ptolemy Project is still recruiting interested participants based on the eligibility requirements. If you match these requirements and are interested in participating in the Ptolemy Project, please let us know by completing and returning the following questionnaire and consent form.
Once we review your questionnaire and consent form, we will send you a letter designating you a Research Affiliate of the Office of International Surgery, University of Toronto and provide you with a username and password to access the University of Toronto Library.
